which is the right approach if you are emabarking on writing a book/thesis?
which tends to happen more in practice?
which is more intuitively obvious?

I'd vote breadth-first happening in practice, because it seems "easier" to think of possibilities. The question "what could we do" is bread-first, no?
Though perhaps breadth isn't something we can explore completely...
